If you'd like to sterilize instruments, you use the ultrasonic cleaner before the autoclave. The ultrasonic cleaner is used to remove all of the stuff you can't see when you wash an instrument - all the stuff that's in places that are hard to scrub yourself or might even miss. If you're just looking for better cleaning, you could just use the ultrasonic cleaner but to sterilize you have to use the autoclave after.
